GENERAL INFORMATION:
o For registration purposes, the age of participants will be considered as of the first day of class.
o Checks are payable to the Middleton Recreation Department
HOW TO REGISTER:
1. Internet
o All programs may be registered for through the Active.com online registration system, a secure site.
o Allows program registrations to be available 24 hours a day, seven days a week.
o There is an additional processing fee for internet registration.
Easy Steps to Complete Online Registration:
i. Find the activities you want to register for.
ii. Go to our website at middletonrec.org and click on programs.
iii. Go to the bottom of the program description page and click on the activity code. This will take you to the active.com site.
iv. Complete the registration information, pay with a credit card, and receive instant email confirmation of your enrollment.
2. Mail
o Use the supplied registration form or include the following information on a piece of paper: Child’s Name, Address, Phone, Age, Class, and Time. Include second choice of time if possible.
o Mail the information, with fees, to the Recreation Department, 7426 Hubbard Ave., Middleton, WI 53562.
o Include a self addressed, stamped envelope to receive class confirmation after processing.
3. After Hours Drop Box
o A drop box is located in the lobby of City Hall, next to the elevator.
o Registration will be processed the next business day.
4. Walk-In
o Registration forms will be accepted in person during normal business hours.
o Complete the supplied registration form or use the forms provided at the City Hall.
o Payment of Fees must accompany registrations.
TRANSFERS BETWEEN CLASSES
o Transfers between classes must be done a minimum of three days prior to the start of a class. After that time, transfers will not be accepted.
REFUNDS
o Full refunds will be made on all programs that are cancelled by the Recreation Department.
o Full refunds will be honored for any written request made a minimum of 5 days prior to the start of a class or program. There will be no exceptions. Refund request forms will be mailed to you upon the Recreation Department being notified.
o Refunds will not be issued for unattended classes, weather related cancellations, or holiday dates.
o A $5.00 service charge is withheld for all refunds, unless the activity is cancelled by the Recreation Department.
o Refunds will only be issued to a credit card account. There will be no monetary refunds issued.
PROGRAM POSTPONEMENTS
o There will be programs on July 4.
o Any program postponed due to inclement weather will not be rescheduled.
AGE OF PARTICIPANTS
o For registration purposes, the age of participants will be considered as of program/class start date.
o To ensure the safety, and quality of programming for our participants, please respect the age requirements for each class or program. Failure to comply with age restrictions may result in the placement of participants into a more appropriate class, or a non-refundable release from the class or program.
PHOTO POLICY
Middleton Recreation Department programs and events are often photographed for promotional purposes. IF you do not wish to be included in a Recreation Department photo, please indicate this to the instructor and/or photographer.
SWIMMING LESSONS
All Recreation Department swimming lessons are taught by Red Cross approved Water Safety Instructors. Classes are offered in accordance with National Red Cross recommendations. Class size will be limited to provide a pupil/instructor ratio that will insure a quality approach to the program. There are NO CLASSES JULY 4th and lessons WILL NOT be rescheduled.
